Monorepo with document storage services on nest.js with apps: rest-api, grpc-api, worker, kafka-consumer

NestJS microservice (API + worker) for timezone-aware appointment reminders. Integrated with MediPulso, it stores appointments in PostgreSQL via Prisma and uses Redis + BullMQ to schedule and deliver email/SMS reminders 24h and 2h before visits, with retries, priorities, and observability (OpenTelemetry, Prometheus, Sentry).
